CHRONOLOGICAL ORDER OF EVENTS


The lamppost was destroyed on impact from a truck.


Description


A truck plate No xx1234 was delivery some wine to the restaurant at 15:30 on 12/12/07. During the worker  done his job and get back to truck, name of CHU Tai Man of truck driver was reserving to turn around and attempting to leave our complex. At that moment, the driver hearing a sound ‘BAND’ and through the truck mirror of see the back of passenger side was joined with a lamppost, the lamppost is 3 m high and install on the pavement and closed to the driving road. Mr.  (security Guard) patrol on the scene discovers the accident and report to management office. Supervisor Mr.  was responded to the scene for investigation, upon KK CHEUNG arrivals he discover the lamppost beside the truck was deformed and damaged, and the truck right back side light also was damaged. Cause by the driver was not co-operating with KK CHEUNG, no personal information was took from the driver, even his name. And the driver also complaint the poor road condition was the reason causes the accident. (The size of road not big enough for the truck easy to turn over).


According to the company policy, policeman was called by security controller. The policeman was attended to the scene for assist after 12 minutes from the call. Cause by the force of policeman, the driver become more co-operate, but he still not agree to sign the statement to recognize his faults and he seems do not want to responsibility for the maintenance payment. The policeman took all information from our supervisor and the driver; the policeman also indicated that if we can not settle the problem in 10 day, any one of us can report to the police station. Before the policeman left, he gave us the reference number for report to the police traffic station and they will takeover the case. The driver will wait for our contact when the amount of maintenance was confirmed by the Engineering Department.


While the truck was leaving our complex, the second accident happened, the truck punch to the barrier cause by when it traveling to the check point, the barrier was not raise up. The security guard duty in the guard house was running out quickly and stops the truck, but it still too late, the barrier can see was completed damaged. Because by the rush traffic condition, the security guard direct the driver park the truck on side without took photo first. Security controller call 999 for assists again, KK CHEUNG arrivals on check point for investigation, for these times, and the driver was much co-operating than before. The driver writing the responsibility letter, and agree will pay for the damaged property. The policeman came back to our complex very soon. The policeman left after the reference number was taken.


Identification of causes after investigation


The incident was cause by the untended driving by bad driving behavior. The loading space is not suit for big truck turn around. Traffic direct should provided to the driver.


CONCLUSION


While any truck access to our complex for any reason, all security guard should pay more attention to direct traffic and alert all vehicles/trucks especially they are reserving.


 


Remark:


After two week, the amount of property damaged was determined. (30,500). the driver tried to ask for reduce the payment but was refused by Management Manager. At last the driver agreed to pay but he has to pay in six month period. He pay about 5000 for each month until the total amount was settle.


 


CHRONOLOGICAL ORDER OF EVENTS


Twp passengers trapped in the elevator.


Description


Security controller received the alarm signal from the elevator, through the CCTV controller saw two ladies was trapped in the elevator, security through the lift intercom to tell the passenger come down and promised will call the lift company for maintenance immediately. Management supervisor KK CHEUNG was attended to the scene for assistance. KK CHEUNG stay outside the lift door and keep talking to the passenger to ensure their health condition was good. The technicians of Lift Company were attended after 18 minutes from the call. Due to the lift was stopping in the middle level between 1/F and 2/F; the technicians should operate the lift to the 1/F level for safety reason. Two ladies were released from the lift after 28minutes from the initial call.


Two technicians were inspecting the lift and left after the lift resume normal. No people report injuries in the incident.


Identification of causes after investigation


This case was an accident.


CONCLUSION


Lift technicians should pay more carefully during they carry out the lift maintenance. To ensure the incident not happen time to time.
