a. The nature of the hazard is the exposed screw terminal and the temperature of the screw. Since there is an exposed terminal and the screws are hot it might result to burning pants because a cloth which is flammable creates friction with the hot screw.  The pant is a flammable substance and since it engages in regular friction with the hot screw the result would be small burning on the pants and the one wearing the pants might get burn a bit.  


b. The nature of the hazard is the ground between the boot and the 440 volt bus bar. Some parts of the boots like its nails and other metal parts conduct electricity with the bus bar and this produces electric surges in the concrete floor. It would not do anything to the wearer of the boot but can cause some electric shocks to those people that are near and not wearing foot protections.


 c. The nature of the hazard is the direct short across the hot terminal and the adjacent neutral. The short between the hot terminal and adjacent neutral creates a hot temperature for the screwdriver shaft and electric shock. It can also conduct some electricity into the screwdriver shaft. But since the handle is wood it won’t do anything to the person using the screwdriver unless he/she accidentally touches the screwdriver shaft.


d. The nature of the hazard is the energized circuit. One who touches an energized circuit should always wear gloves and other protection. The worker changing the wall receptacle doesn’t have the assurance that nothing will happen while holding the hot and neutral wire. One false move and the hot and neutral wires might touch and a small explosion might happen. It could also result to electrocution to the worker or other people near him/her.

a. A hot work permit requires safety precaution before, during and after work. The safety precaution will remind the workers about what they should remember before working, during work hours and after working. A hot working permit requires information such as date issued, validity, building number, location of work and contractor contact numbers. The hot work permit contains a part where the responsible party for a certain action would have to put his/her signature beside the date.


b. Traditionally, occupational injuries and accidents have been viewed as singular system outcomes independent of profitability, management, or organizational context. Most organizational strategies for dealing with safety issues create one or two psychological consequences. The first psychological consequence is to increase workers’ knowledge. The most common technique is to train people on proper techniques, machine use, or safety procedures. Other techniques for increasing knowledge include creating greater awareness, alerting people to the environment, providing cautions, and introducing new information (Hendrick, & Kleiner, 2002). Safety in the workplace is not only an important social issue; it is a concern for business because an unsafe work place can put the employee’s life at stake. No employee would want to work in such condition. Safety devices such as a hot work permit could have prevented the accident through issuing important reminders that should be remembered before doing any activity. The hot work permit can give the workers a proper training on how to handle the materials and proceed with their task without causing problems to other people.
